Doctors in Ontario are advising immediate medical care after any direct contact with a bat, even without visible bites or scratches, following the death of an 11-year-old boy. The child awoke to find a bat on his face but showed no wounds, and his parents saw no unusual behavior in the animal, so they did not seek treatment. Nineteen days later he developed facial numbness, vomiting and pain, leading to a rabies diagnosis. With symptoms present, only supportive care was possible. This marked the first locally acquired rabies case in Ontario since 1967. Canada has recorded just 28 rabies cases since 1924. The parents approved publication of the case in the Canadian Medical Association Journal to increase awareness. Medical staff noted that bat bites or scratches are often tiny and unnoticed. Any direct contact with a bat warrants post-exposure prophylaxis after consultation with public health officials. Rabies attacks the nervous system through infected saliva and is nearly always fatal once symptoms start. Prevention relies on prompt post-exposure prophylaxis combining antibodies and vaccine before symptoms appear. The incubation period varies from days to months, and early signs such as tingling, fever or headache can mimic other illnesses. In North America bats are the main source, as their small teeth and claws leave wounds easily missed. The boy first showed vomiting and facial numbness, was initially treated for a mouth infection and sent home, then returned with fever, confusion, swallowing difficulty, excess saliva and hallucinations. He received intensive care but declined rapidly. Life support was withdrawn on day 17 and he died with family present. Finding a bat in a room alone does not require prophylaxis, yet any possible contact means the animal should be tested if captured. The case report urges anyone with direct contact to seek care immediately regardless of the bat’s appearance.
