Kerala officials issued two notices to the contractor of the Anakkampoyil-Kalladi-Meppadi tunnel project between June 20 and 25. The directives required removal of excavated soil from the location. The Wayanad district collector ordered a temporary suspension of work near Meenakshi until the material was cleared. Operations continued, resulting in a collapse that caused three deaths, several injuries and five people missing. The state environment minister called the event a preventable incident caused by disregarded instructions and said lapses are under review.
