At elevations above 3,500 metres on the Tibetan Plateau, residents have developed physiological responses to reduced oxygen levels over many generations. Communities maintain normal activities such as agriculture and family life despite conditions that cause discomfort for visitors. These adjustments appear in blood chemistry, heart performance, and oxygen transport within red blood cells rather than through one major alteration.

A study published in PNAS examined ethnic Tibetan women living at high altitude in Nepal. It found that moderate haemoglobin concentrations combined with efficient oxygen delivery often correlated with higher lifetime reproductive success. Elevated blood thickness can increase strain on the heart, so balanced saturation levels appear more advantageous for sustained function.

Reproductive outcomes served as an indirect measure of adaptation. Women displaying certain cardiovascular traits recorded more successful pregnancies and births. These patterns suggest that beneficial traits become more common across generations without implying a single directed process. Social factors including family timing also influence results.

Additional observations include modestly enlarged left ventricles in some individuals and improved pulmonary blood flow. Together these traits support better oxygen extraction from each breath while limiting cardiovascular stress. The findings highlight measurable physiological variation linked to long-term residence at extreme altitudes.
